ABSTRACT

Objective:To evaluate the clinical efficacy of channel screw guided by O-arm navigation for the treatment of type II fragile fracture of pelvis (FFP) in the elderly.Methods:A retrospective cohort analysis was performed on clinical data of 37 patients with type II FFP admitted to Shanghai Pudong Hospital, Fudan University Pudong Medical Center from September 2019 to April 2021. There were 9 males and 28 females, aged 65-82 years [(71.8±10.1)years]. A total of 15 patients receipt channel screw fixation under O-arm navigation (surgical group) and 22 patients were treated conservatively (non-surgical group). The visual analogue scale (VAS) and Majeed functional score were compared between the two groups at emergency visit and at 1 week, 1 month, 3 months, 6 months after treatment. The accuracy of screw insertion and complications were also recorded during treatment and follow-up.Results:All patients were followed up for 6-18 months [(13.2±5.1)months]. There were no significant differences in the VAS and Majeed functional score between the two groups at emergency visit (all P>0.05). In both groups, lower VAS and higher Majeed functional score were found at 6 month after treatment as compared with those at emergency visit (all P<0.01). The VAS in surgical group was 2(1, 4)points at 1 week after treatment, lower than 4(3, 5)points in non-surgical group ( P<0.01). The VAS in surgical group was 1(1, 2)points at 1 month after treatment, lower than 3(2, 5)points in non-surgical group ( P<0.05). The Majeed functional score in surgical group was (50.2±4.2)points at 1 week after treatment, higher than (40.2±5.6)points in non-surgical group ( P<0.01). The Majeed functional score in surgical group was (73.8±5.2)points at 1 month after treatment, higher than (62.4±5.0)points in non-surgical group ( P<0.01). The two groups had no significant differences in VAS and Majeed functional score at 3 months and 6 months after treatment (all P>0.05). The accuracy of screw insertion in surgical group was 93% (14/15). In surgical group, the complication rate was 13%(2/15), including urinary tract infection in 1 patient and intraoperative screw penetration in 1, with no screw loosening or nerve and vascular injuries. In non-surgical group, the complication rate was 50%(11/22), including urinary tract infection in 5 patients, penetrating pneumonia in 5 and deep vein thrombosis in 1. The complication rate was significantly different between the two groups ( P<0.05). Conclusions:Compared with non-surgical treatment, the channel screw insertion guided by O-arm navigation can achieve precise screw placement, relieve pain early, promote functional recovery, realize accurate insertion of the screw and reduce the complication rate for type II FFP in the elderly.

ABSTRACT

Dendrobium(Orchidaceae) is traditional Chinese medicine with a high healthcare value,which can nourish Yin and tonify deficiency. Eighty-nine flavonoids were isolated from Dendrobium,mainly including flavones,flavanoes and flavonols. Among them,there were 40 flavonoids,the main aglycones were apigenin and chrysoeriol;20 flavanones;and 15 flavonols,and the main aglycones were kaempferol and quercetin. D. officinale and several other species also have flavanonols,anthocyanidins,chalcone and isoflavones. There were 34 species of Dendrobium containing flavonoids,including 38 flavonoids in D. officinale,28 flavonoids in D. huoshanense,19 flavonoids in D. devonianum,12 flavonoids in D. wardianum,5 flavonoids in D. thyrsiflorum,4 flavonoids in D. denneanum and D. findlayanum. Common flavonoids included naringenin,quercetin,rutin,which had pharmacological effects of resisting oxidation,lowering blood sugar,improving blood circulation,lowing cholesterol,and protecting the cardiovascular system. The existing studies of Dendrobium-related species,flavonoids and their physiological functions were reviewed in the expectation to promote the attention of the industry to Dendrobium flavonoids and explore values of Dendrobium plants in efficacy and food fields.

BACKGROUND@#Zika virus (ZIKV) has emerged as a global pathogen causing significant public health concerns. China has reported several imported cases where ZIKV were carried by travelers who frequently travel between China and ZIKV-endemic regions. To fully characterize the ZIKV strains isolated from the cases reported in China and assess the risk of ZIKV transmission in China, comprehensive phylogenetic and genetic analyses were performed both on all ZIKV sequences of China and on a group of scientifically selected ZIKV sequences reported in some of the top interested destinations for Chinese travelers.@*METHODS@#ZIKV genomic sequences were retrieved from the National Center for Biotechnology Information database through stratified sampling. Recombination event detection, maximum likelihood (ML) phylogenetic analysis, molecular clock analysis, selection pressure analysis, and amino acid substitution analysis were used to reconstruct the epidemiology and molecular transmission of ZIKV.@*RESULTS@#The present study investigated 18 ZIKV sequences from China and 70 sequences from 16 selected countries. Recombination events rarely happens in all ZIKV Asian lineage. ZIKV genomes were generally undergone episodic positive selection (17 sites), and only one site was under pervasive positive selection. All ZIKV imported into China were Asian lineage and were assigned into two clusters: Venezuela-origin (cluster A) and Samoa-origin cluster (cluster B) with common ancestor from French Polynesia. The time of most recent common ancestors of Cluster A dated to approximately 2013/11 (95% highest posterior density [HPD] 2013/06, 2014/03) and cluster B dated to 2014/08 (95% HPD 2014/02, 2015/01). Cluster B is more variable than Cluster A in comparison with other clusters, but no varied site of biological significance was revealed. ZIKV strains in Southeast Asia countries are independent from strains in America epidemics.@*CONCLUSIONS@#The genetic evolution of ZIKV is conservative. There are two independent introductions of ZIKV into China and China is in danger of autochthonous transmission of ZIKV because of high-risk surrounding areas. Southeast Asia areas have high risk of originating the next large-scale epidemic ZIKV strains.

Objective@#To evaluate the minimally invasive splayed incisions in the internal fixation with a conventional calcaneal plate for calcaneal fractures of sanders types Ⅱ and Ⅲ.@*Methods@#This prospective study was conducted from May 1st, 2016 to December 1st, 2017 in the 40 patients with calcaneal fracture at Department of Orthopedics, Shanghai Pudong Hospital. Their ages ranged from 23 to 55 years (average, 39.5 years). According to the Sanders classification, 27 fractures were type Ⅱ and 13 type Ⅲ. They were all treated with a conventional calcaneal plate through minimally invasive splayed incisions. The Böhler and Gissane angles, the height, width and length of the affected calcaneus were compared between preoperation, 3 months after operation and the last follow-up; the clinical function of the affected feet was graded using the Maryland foot score; postoperative complications were observed.@*Results@#The 40 patients were followed up for an average of 12.5 months (from 11 to 16 months). All the skin incisions healed well with no skin necrosis or wound infection. No injury to the sural nerve occurred. All the fractures healed after an average of 8 weeks (from 7 to 10 weeks). All the patients resumed their routine daily activities and returned to their former work post after an average time of 4.1 months (from 3 to 6 months). At pre-operation, 3 months after operation and the last follow-up, their Böhler angles were respectively 19.2°±6.3°, 30.5°±6.4° and 29.9°±6.5°; their Gissane angles 103.9°±14.8°, 119.3°±5.6° and 119.8°±6.3°; their calcaneal heights (32.5±3.5) mm, (36.8±1.5) mm and (36.5±1.8) mm; their calcaneal widths (36.8±3.4) mm, (33.1±3.8) mm and (33.0±3.2) mm; their lengths (61.4±4.5) mm, (65.5±6.9) mm and (65.5±9.4) mm. In all the patients, the Böhler and Gissane angles and the calcaneal heights and lengths increased significantly while the calcaneal widths decreased significantly at 3 months after operation and the last follow-up (P<0.05). There were no significant differences between 3 months after operation and the last follow-up in the Böhler or Gissane angle, the height, width or length of the affected calcaneus (P>0.05). Their Maryland foot scores showed 35 excellent cases, 4 good cases and one fair case, giving an excellent and good rate of 97.5%.@*Conclusions@#A conventional calcaneal plate plus minimally invasive splayed incisions can be effective for calcaneal fractures of Sanders types Ⅱ and Ⅲ, leading to reduced wound complications, anatomical restoration of calcaneal morphology, and smooth subtalar articular surface.

ABSTRACT

Alkaloids were regarded as important biological activity constituent in medicinal Dendrobium Sw., being characteristic pharmacodynamic components in plants from Dendrobium Sw. A total of 52 constituents of alkaloids were found in plants of Dendrobium Sw. They could be divided into five types, according to different structures. They were sesquiterpenoid alkaloids, octahydroindolizine alkaloids, amides alkaloids, pyrrolidine alkaloids, and imidazole alkaloids etc. Sesquiterpenoid alkaloids could be divided into dendrobine type, dendroxine type, nobiline type, etc. Over 19 Dendrobium species contain alkaloids, and the main species included D. nobile, D. chrysanthum, D. crepidatum, D. findleyanum, D. signatum, D. friedericksianum, etc. This paper reviews the structural classification of alkaloids in Dendrobium, and the species which containing alkaloids and the influencing factors of alkaloids in Dendrobium Sw., in order to provide reference for the in-depth study and the development of alkaloids in plants from Dendrobium Sw.

ABSTRACT

Objective To investigate the efficacy of low-profile anatomical buttress plate in treating vertical shear medial malleolus fractures.Methods A retrospective case series study was conducted on the clinical data of 20 patients with vertical shear medial malleolus fractures admitted from February 2013 to February 2015.A total of 13 males and seven females were included in the study,with an average age of 45.8 years.There were 11 patients with medial combined with lateral malleolus fractures,one with medial malleolus fracture,six with medial combined with posterior and lateral malleolus fractures,and two with medial combined with distal tibial fractures.All patients were treated with low-profile anatomical buttress plate.Bone union,reduction loss,internal fixation stability,and incidence of ostearthritis were recorded.The postoperative ankle function was evaluated according to American Orthopedic Foot and Ankle Society (AOFAS) score.Results All patients were followed up for26.5 months on average (range,18-37 months).Anatomical reduction was seen in all patients according to X ray.Patients were seen bone union after an average of 15.6 weeks.After 8-16 weeks,patients were able to walk and return to work,averaging 12.5 weeks.Patients were able to do physical exercise after 20-26 weeks (mean,22.5 weeks).The follow-up showed no complications such as reduction loss,internal fixation loosening or breakage.Bone nonunion was not seen,and no manifestation of osteoarthritis was shown in the images.An average AOFAS score of 88.6 points (range,75-100 points) was reported at the final follow-up.The scoring outcome was excellent in 13 patients and good in seven,with excellent and good rate of 100%.Conclusions The low-profile anatomical buttress plate for fixation of vertical shear medial malleolus fractures has advantages of firm fixation,early mobilization and sound function recovery.The plate can be a safe and effective method for the treatment of vertical shear medial malleolus fractures.

ABSTRACT

@#BACKGROUND: The study aimed to investigate the therapeutic benefits of intravenous Xuebijing on acute kidney injury (AKI) in rats with paraquat intoxication. METHODS: Male Sprague-Dawley rats were randomly divided equally into three groups:sham group (n=8), paraquat group (n=8) and Xuebijing-treated group (n=8) using a random number table. The rats were intraperitoneally injected with 50 mg/kg of paraquat. One hour after paraquat administration, the rats were treated intravenously with Xuebijing (8 mL/kg). At 12 hours after paraquat administration, serum was collected to evaluate kidney function, then the rats were sacrificed and kidney samples were immediately harvested. AKI scores were evaluated by renal histopathology and pro-inflammatory cytokines mRNA levels in kidney were assayed using real-time RT-PCR. RESULTS: Serum urea nitrogen, creatinine and AKI scores were significantly higher in the paraquat group, compared with the sham group (P<0.05, respectively). Moreover, interleukin (IL)-1β, IL-6 and TNF-α mRNA levels were significantly higher in the paraquat group (P<0.01, respectively). However, intravenous Xuebijing significantly decreased serum urea nitrogen, creatinine, AKI scores and IL-1β, IL-6 and TNF-α mRNA levels, compared with the paraquat group (P<0.05, respectively). CONCLUSION: Intravenous Xuebijing attenuates AKI fol owing paraquat poisoning by suppressing inflammatory response.

ABSTRACT

Intellectual disability is defined as a group of diseases with generally mental function significantly lower than the same age level, accompanied by adaptive behavior defectiveness in a developmental stage. Hyperactivity is a common symptom of intellectual disability in children. In addition to clinical rehabilitation training, TCM treatment for this disease also achieved better efficacy. This article expounded tonifying essence and blood, promoting coronary circulation and nourishing the brain, and tranquilizing through the thoughts of classification treatment of viscera classification syndromes, the treatment of meridians, and separate treatment for san jiao, and combination of acupuncture and TCM, which have achieved satisfactory efficacy.

ABSTRACT

There are 250 species of Zanthoxylum (Rutaceae) in the world. This genus distributed in tropical and subtropical regions. Alkaloids are the major and representative ingredients in these plants including quinolines, isoquinolines, and amide alkaloids, with such biological activities as anti-tumor, anti-inflammatory, analgesic, anti-virus, anti-platelet aggregation, anti-bacteria and anti- oxidant. These species have been used for a long time to treat toothache, urinary and venereal diseases, lumbago and rheumatism. This review summarizes the chemical constituents and pharmacological activities from the Z. sppplants, in an effort to the systematic research and application of the alkaloids of this genus.

ABSTRACT

<p><b>OBJECTIVE</b>The study was conducted to reveal the distribution of genetic polymorphism of four Y chromosome specific short tandem repeat (Y-specific STR) loci in Li ethnic groups in Hainan Island, China.</p><p><b>METHODS</b>Four tetranucleotide STR loci were simultaneously amplified with fluorescently labeled primers, and genotypes were determined with an automated DNA sequencer.</p><p><b>RESULTS</b>Among 230 unrelated males, the alleles at the four Y-specific STR loci were composed of some complex repeat structure. 4,5,4,5 alleles were observed in loci DYS3891, DYS390, DYS391, DYS393 respectively. A set of human allele ladders for the typing of the four Y-specific STRs was obtained in Li ethnic population. Gene diversity index (D) and haplotype diversity data were estimated for the four Y-STRs.</p><p><b>CONCLUSION</b>The preliminary study indicates a reference population for detecting male migration events and should be useful in population genetics and forensic applications.</p>

ABSTRACT

50?mol/L induced markedly oncotic cell death,but no apoptosis was observed.TEM examination indicated that a form of cell death accompanied by cellular swelling,organelle swelling and vacuolization,mitochondrial swelling and cristae membrane loss,and nucleus swelling, chromatin scattering or karyolysis,which characterized as oncosis.When treated with 50,200?mol/L of ART for 5 h, the intracellular ROS level of Panc-1 cells markedly increased to 1.60 and 4.49 fold compared with that of untreated cells,respectively.Pretreatment with TCEP effectively attenuated ART-induced intracellular ROS level and decrease the oncosis in Panc-1 cells.Conclusions:ART exerts profound cytotoxic effects on Panc-1 cells and induces an oncosis- like cell death,which is quite different from apoptosis.The cellular generation of ROS and its peroxidation damage may be one of the mechanisms for its anti-tumor effect on pancreatic cancer.
